Please complete all of the green parts of your Order Form. Your full name and amount of children must be on each order form.
Items are listed like a vending machine. The letter indicates the price, and the number indicates your selection.
You may purchase for up to 6 children in your household. You may select up to 4 gifts per child. For each gift, you will write the Item Code and Item Name on your Order Form (Ex: A3 - Endzone Challenge Game).
You may select ONE A item or ONE B item, but not both.
Once you have completed your Order Form, please bring it to your program leader to be checked and submitted. You will receive your total today and payment will be due on Pick-Up Day.
